Eligible applicants include: Lead Local or Tribal Health Officials (Health Officer, Public Health Director) and Board of Health Chairs.
- Ideally, the lead local health official and their Board of Health Chair will attend together but not required.
- Both individuals from the same local health jurisdiction can apply for scholarship assistance.
- Please only complete one application per local health jurisdiction.
