The Public Health System Improvement Office (PHSIO) is offering a series of webinars to provide an overview of how to complete a Community Health Assessment (CHA) and a Community Health Improvement Plan (CHIP). These are voluntary processes undertaken by Local and Tribal Health Departments that are rolled up to be included in the State Health Assessment, and are very similar to the assessment and planning processes required of non-profit hospitals.
